Patients are seen by appointment only.
Please be prepared with your insurance card information. You will need to bring your insurance card, photo identification, and a list of current medications when you come in.

New patients will have several forms to fill out.
Please come in 10 minutes before your appointment time, or feel free to download the forms, and fill them out before you come in to the office.

Hours

The ENT Group of Cleveland and Westshore Hearing Center are open Monday, Tuesday, Thursday & Friday in Westlake.
The office is closed between 12:00 and 1:00 for lunch.
Office hours will vary occasionally due to surgery scheduling.
Phone Calls

Please limit non-emerency calls to the regular office hours.
Emergency calls during non-office hours will be routed through the answering service.
Scheduling Appointments

Patients are seen by appointment only. Please be prepared with your insurance card information when calling to schedule an appointment. Keep in mind that some insurance companies do require a referral from your primary care physician, as well as pre-authorization in order for your visit to be covered. You will need to bring your insurance card, photo identification, and a list of current medications when you come in. Insurance

The ENT Group of Cleveland accepts most major commercial insurances, as well as Medicare and Medicaid. Self-pay patients are asked to pay for services on the day of the visit, unless special arrangements have been made. We accept Master Card, Visa, and Discover.
Fees

Examination fees will vary depending on the nature of the visit, as well as addtional testing or procedures required for proper diagnosis and treatment.
